Pre-Register

You may print out the registration, history forms and HIPAA forms on this page.  Complete the front and back of forms as appropriate and bring them with you to your appointment.  You will still need to arrive approximately 15 minutes prior to your appointment time for us to get your information entered and your chart made.  Please also, remember to bring your drivers license and insurance cards to your appointment.
