Insurance

We are participating providers with many major insurance companies such as, Medicare, Blue Cross and Tricare etc.  However, we are NOT participating providers for United Healthcare or Medicare Complete.  If your insurance requires you to have a referral from your primary care physician (PCP), please contact your PCP as soon as you know your appointment date so they may get the referral to our office in a timely manner.  When coming in for your appointment, please bring your drivers license and your insurance cards with you.  This will give us photo identification as well as all pertinent information to bill your insurance company for your visit.  Please go to Insurance Referrals for more information.
